Homeless Healthcare Team
and Special Allocation Scheme
There is a recognised population of homeless people and families across the county who find it difficult to access primary health care because of their circumstances and lifestyle.
Our Homeless Healthcare Team offers permanent registration for those patients with a high level of need, and provides a safe environment for patients who have been removed from GP lists and are registered on the SAS scheme.
We provide a flexible service which enables homeless people to access primary health care.
Our aim is to promote self-care, whilst improving access to mainstream health and social care services, with the ultimate goal of re-integrating people back into mainstream primary health care.
