What is Home First and Reablement?
Our Home First and Reablement service supports people to return home after a stay in hospital. This helps them continue their recovery in their own surroundings – regaining their ability to live as independently as possible.
Staying in hospital for longer than necessary has a negative impact on how well patients recover. Ensuring that patients are given the chance to continue their lives at home is vital for their long-term health and wellbeing.
The support we provide includes washing, dressing, cooking meals, accessing community services, arranging delivery services, improving mobility and balance so patients can safely use steps or stairs, walk in and outdoors, or use transport. They may have a particular hobby or interest that is important to them and would like help to get back doing the things they enjoy – this is something you can help them achieve.
What is reablement?
The role of reablement is to support people to lead independent lives in their own homes, safely and appropriately, for as long as possible, by enabling them to do things for themselves instead of relying on others to do things for them.
Typically, reablement support is given to people following an illness, injury or other sudden event which may have reduced their physical, emotional or psychological ability to manage their own lives.
Our reablement workers receive specialised training and work in the area as part of our integrated community teams (ICTs).
Reablement is delivered in partnership with the service user, as the reablement worker and the individual both have responsibilities in working towards recovery and agreed goals.
Together we support people to be in control of their choices and to maintain their independence safely and appropriately. We also ensure to refer on for specialist care when it is needed.
When will I first see a reablement worker?
It varies, depending on your individual needs. In an urgent situation you may receive a rapid response visit in less than one hour.
A planned response to coincide with your discharge from hospital would take place within three days.
First contact might be made by any member of the integrated community team – for example, a community nurse or physiotherapist – depending on your needs and the support you require.
How often will a reablement worker visit me?
Reablement is offered as a short-term intervention – the actual length of time the support is provided depends on your individual needs.
Visits will be planned with you and might be several times a day initially, reducing over time as you begin to recover your independence.
How do I contact my reablement worker?
If you have a first enquiry about reablement for yourself or a member of your family, your GP is often the best person to advise you.
You can also contact the Adult Social Care Helpdesk, which is run by Gloucestershire County Council, and advises on a range of issues. The Helpdesk is open 8am–5pm, Monday–Friday, and can be contacted on 01452 426868.
When you are visited for the first time you will be provided with the name and contact details of your local integrated community team (ICT), who will coordinate your care.

How will my family/carers be involved?
Your family/carers will be involved as much or as little as you want them to be. Agreeing how you want to be supported will include consideration of your relationships with family, carers and friends. Your carers will also be offered an assessment of their needs.

Rehabilitation Assistants
Rehabilitation Assistants work as part of a our therapy led team, undertaking activities according to the person’s goal plans, encouraging them to do as much as possible for themselves and live well and safely in their own homes. Rehabilitation Assistants do this by motivating and inspiring people to re-learn daily skills and enhance their quality of life.
Are you an enthusiastic and motivated person who wants to take up a new role working with people in their own homes? We can offer you training, development opportunities, regular supervision and support.

Successful applicants will have the opportunity to discuss the prospect of undertaking a Level 3 Healthcare Support Worker Apprenticeship alongside their substantive post, this apprenticeship would include the Level 3 Diploma in Clinical Healthcare.

Physiotherapists

Opportunities exist for you to work with rapid access from our referral centre, bed-based reablement, Home First and domiciliary intervention so there’s plenty of scope for your professional development.

Occupational Therapists
We are looking for Occupational Therapists who have experience of rehabilitation, excellent communication skills and are good team players. In return we will support you in developing you leadership, line management and clinical skills.
Working in our multi-disciplinary team involves both triage and assessment of patient need, as well as a caseload of patients in community reablement. It is a multifaceted role involving both triage and urgent response for rapid assessment of patient need, as well as a varied caseload of patients.
