Children’s Community Nursing
Our Children’s Community Nursing Team (CCNT) provides a community-based nursing service for children and young people, up to the age of 18, with specific medical conditions requiring nursing treatment.
About this service
It is a countywide specialist nursing service for children and young people who have a designated consultant paediatrician managing their ongoing care. We provide support in the most appropriate place for each individual. This may include schools, clinics, nurseries or in the child’s home.
We help to support early discharge from hospital or prevent hospital admission where possible. Our nursing care will enable our patient’s individual acute and complex health needs to be managed in their own home or in a clinic setting, where appropriate. We also provide transitional care for those moving into adult services.
We specialise in:
- Gastrostomy management
- Nasogastric care
- Wound care
- Assessment for specialised bed and gastrostomy equipment
- Intravenous medication
- Subcutaneous injections for conditions
- Cardiac care following discharge from regional centre
- Venepuncture
- End of life care to children in their own homes.
Referrals
We accept referrals from GPs, consultants or other healthcare professionals such as hospitals or other agencies.
All children and young people referred to the Children’s Community Nursing service must have a designated consultant paediatrician managing their on-going care.
